  • Assistance with Owensboro Health Financial Assistance applications
  • Guidance through Medicare coverage options
  • Applications for Medicare Savings Programs, Kentucky Medicaid, and Extra Help for Part D

Survivorship Plan

After completing cancer treatment, ask your physician or navigator if a survivorship plan is available for your type of cancer. Your personalized plan will outline:

  • Recommended tests and screenings to monitor your health
  • Healthy habits to maintain
  • Symptoms and signs to watch for

It also summarizes your treatment and lists your care providers. Share your plan with your primary care doctor to support long‑term wellness.
